Nursing Homes (Skilled Nursing Care)

Mercyhealth’s two nursing home locations—Mercyhealth Care Center and Mercyhealth Transition Center — offer around-the-clock skilled nursing care for individuals recovering from illnesses and injuries.

At our Mercyhealth Care Centers, our compassionate rehabilitation staff focuses on easing our patients’ transition from the hospital to the comfort of their home or other living arrangements. We do that through our effective short-term rehabilitation program, known as skilled nursing care. This phase of care follows the patient’s hospitalization due to illness, surgery or general medical decline in physical condition.

Health problems like stroke, congestive heart failure, lung disease, joint replacements, hip fractures can affect the entire body. Our qualified rehabilitation experts approach each individual with a whole-person and specialize in pain management, wound care, physical therapy, occupational therapy and speech therapy.

If special needs arise, both locations offer patients great peace of mind due to quick, down-the-hall access to hospital resources and health care professionals.
