Mercyhealth earns honor for improving patient health outcomes

Recognized with 2015 UnitedHealthcare PATH Excellence in Patient Care Service Awards

Mercyhealth was recently recognized with the 2015 UnitedHealthcare PATH Excellence in Patient Service Award for its commitment to improving health outcomes for patients enrolled in UnitedHealthcare’s Medicare Advantage plans.

Mercyhealth receives the award for achieving performance metrics in the UnitedHealthcare PATH Program and successfully addressing care opportunities when treating UnitedHealthcare’s Medicare Advantage members. The program annually rewards physicians who meet certain performance-based criteria, including achieving or exceeding benchmarks established by the Centers for Medicare & Medicaid Services (CMS) for specific Healthcare Effectiveness Data and Information Set (HEDIS) measures.

Mercyhealth was evaluated based on its percentage of UnitedHealthcare Medicare Advantage members who:

  • Received a breast or colorectal cancer screening
  • Live with diabetes and also received an eye exam
  • Received advice from their doctor to maintain or enhance their level of physical activity
